Hip pain rarely has one simple cause. The joint itself, the surrounding tendons, the bursae, and even the lower back can all contribute to what feels like a single problem. Here are the most common culprits Dr. Sandy sees at Key to Health.

Hip Osteoarthritis

The most common cause of chronic hip pain. As the cartilage inside the hip joint wears down, bone begins rubbing against bone. The result is deep, aching pain that gets worse with movement and stiffens up after sitting or sleeping.

Bursitis

The fluid-filled sacs that cushion the hip joint can become inflamed from repetitive motion, prolonged pressure, or injury. Trochanteric bursitis in particular causes sharp pain on the outer hip that makes walking and lying on that side miserable.

Labral Tears

The labrum is the ring of cartilage that lines the hip socket and keeps the joint stable. When it tears, patients experience deep groin pain, clicking, and a feeling of the hip catching or locking during movement.

Tendinopathy

The tendons connecting the hip muscles to the bone can develop chronic degeneration from overuse, aging, or injury. Unlike acute tendon tears, tendinopathy builds slowly and tends to be stubborn and slow to heal.

Hip Impingement (FAI)

Femoroacetabular impingement occurs when bone spurs develop on the hip joint and create abnormal contact during movement. It causes pain and stiffness in the groin and front of the hip, especially during rotation.

Referred Pain from Back

Not all hip pain originates in the hip. Lumbar disc problems and sacroiliac joint dysfunction frequently refer pain into the hip and glute region, which is why treating the hip alone often doesn’t produce lasting relief.

Temporary Relief Isn't the Same as Healing.

Hip pain tends to be one of the more treatment-resistant conditions because of its complexity and the limited blood supply to cartilage and joint tissue. Here’s where most approaches fall short: 

Medication

NSAIDs and pain medications manage the inflammatory response but do nothing to repair cartilage, heal tendons, or address labral damage. Long-term use carries its own risks and the pain reliably returns when the medication stops.

Cortisone Injections
A cortisone shot into the hip joint can provide several weeks or months of relief but the effect diminishes with each subsequent injection. More importantly, repeated cortisone use has been linked to accelerated cartilage breakdown, which makes the underlying condition worse over time.

Physical Therapy
Strengthening the muscles around the hip can reduce stress on the joint and improve function significantly. But PT doesn’t regenerate cartilage, heal a torn labrum, or resolve chronic tendon degeneration at the cellular level.

Surgery
For severe osteoarthritis, hip replacement is often the end destination. But it’s a major procedure with a lengthy recovery, real surgical risk, and an implant that has a finite lifespan. Many patients want to delay or avoid it entirely, and SoftWave gives them a meaningful option for doing that.

Most sessions take around 15 minutes from start to finish.
